How Contracted Service Coordination Works
1. Upon Admission
- Substantial effort is taken to assign a primary service coordinator to a consumer - taking into consideration personality, demeanor, communication style, etc.
- An initial meeting is scheduled with the consumer and any involved parties of the consumer’s choice within the first 20 working days of referral.
2. Initial Service Coordination
- The consumer file is reviewed and updated; pending and critical services are prioritized.
- All financial documentation and functions are reviewed to assure funding contracts and agreements are current.
- Consumer assessments are conducted, updated, and renewed where appropriate.
- A plan is developed in collaboration with the consumer and any and all other appropriate parties – focusing upon best services for allocated dollars.
3. Ongoing Service Coordination
- The coordinator maintains regular contact with the consumer to assure that the plan remains consistent with consumer needs, preferences, and the funding allocated.
- The consumer plan is regularly monitored and modified as needed.
- The coordinator regularly monitors services and dollars afforded the consumer and makes changes and adjustments as may be necessary.
