A Service Coordination Program For Adults Relocating From Institutional Settings To The Community

What Is The Goal Of Relocation Service Coordination?

Relocation Service Coordination assists adults, generally under age 65, moving from restrictive environments (i.e. nursing home) to those more suitable to achieving the highest levels of independence.
We are committed to finding the best possible placement for all consumers with whom we work. Together with the consumer, we plan and arrange for any of the following services, including but not limited to:
  • Vocational
  • Psychological
  • Legal
  • Community
  • Health & Beauty
  • Nutritional & Food Services
  • Advocacy
We promote the highest level of individual choice possible for all consumers. As such, we work in conjunction with a wide variety of outside agencies, programs, families, friends, and individuals to help each consumer obtain and maintain as independent a lifestyle as possible.

How Relocation Service Coordination Works

  • Each consumer is assigned a primary case worker responsible for coordination of relocation services. The case worker conducts assessments and consumer interviews to determine housing and service needs. Next, they develop an individual relocation service plan for the consumer.
  • The case worker then works in conjunction with all parties and agencies necessary for implementation of the plan. They also coordinate referrals to set up medical, social, financial, psychological, and other necessary services and supports.
  • Next, the case worker reviews plan implementation and service delivery to ensure that the consumer will have all services necessary upon relocation.
  • When all the above is in place, the case worker coordinates with the institution discharge planner to arrange implementation of the service plan.
  • Lastly, the case worker follows-up with the consumer to ensure that all services are being delivered. Together with the consumer, the case worker evaluates the effectiveness of the relocation service plan and makes any changes necessary.

What Are The Consumer Benefits Of This Program?

  • Independence: The consumer will be living in a less restrictive environment. They will also be an active participant in planning and executing the move as well as choosing services.
  • Social Skills: The consumer will increase social skills by increased usage of community services (i.e. public transportation, libraries, post offices, banks, stores, etc). They will also increase their participation in social/recreational activities due to increased availability. Finally, they will be an active participant in making decisions and understanding the benefits and consequences of the decisions they make.
  • Financial Skills: The consumer will have increased financial independence through money acuity and handling. They will also have an increased awareness of individual funding mechanisms (Economic Assistance and Social Security).
  • Mental Health: The consumer will gain self-esteem and improved mental health through re-establishing independence.
